Module content:
The module presents an opportunity for a close critical analysis of the National Service Framework for Long Term Conditions and other relevant Government policies to examine the ways that health and social care services are delivered. All aspects of the practitioner role will be considered, from commissioning negotiation to service delivery. Case management approaches (as reviewed in the previous level 3 module) will be considered alongside other modalities of service intervention, including rehabilitation and re-enablement. Students will be encouraged to explore methods of evaluation, outcome measurement and improvement practices in the context of user and carer empowerment and quality of life impact, as defined by the world health organisation. Integrated service delivery will be promoted and practitioners invited to consider user experiences, self management and personalisation.
What will I learn?
• Critically reflect on person centred care considering the user and carer experience
• Critically evaluate case management within a multi professional context.
• Critically reflect on the practitioner role and implementation of the personalisation agenda within own area of service delivery.
• Construct a strategy for implementing a change in service delivery.
• Critically evaluate the implementation of Government policies affecting people with long term conditions and the effect on service and individual care delivery.
How will I be assessed?
Formative Assessment: Present a 1,000-word case study highlighting the key facets of the service experience of a person with a long term condition. Use feedback from staff and peers to inform the assignment.
Summative Assessment: One 5,000-word assignment to design and critically evaluate the implementation of an improvement in service delivery and consider the impact this will have on people with a long term condition and their carers.
